Student ID Request Form

CSUCI Extended University recognizes the possible community and local benefits our online students may benefit from having a physical student ID.

A student ID is not required.

Graduate/Master's Degree Program Student ID Request Form

Upon submitting your request, Student Business Services (SBS) will send an invitation for online photo submission via email and a $15.00 charge will be applied to your account. ID will be printed and mailed to students.
